Three men died when a Cessna 210 spun into the water near the Murray Mouth in February. The Australian Transport Safety Bureau's final report, released on Wednesday 28 October, says the instructor likely put the plane into a spin it was never approved to do, and that other pilots knew he'd done it before but nobody reported it.
